While the 2009 edition says that it was first published in the U.S. by Carlton International Media, the book copyright is 1969 and the television series copyright is 1967 by Independent Television Ltd (ITV), and, although one source says it was first published in hardcover in 1967, I have been unable to identify a copy of the 1967 publication.
